Required Fields:
Last Name - Must match exactly, no partial name searches, not case sensitive.

SSN - Last four digits of your Social Security Number, must match exactly and be numeric.

Birthdate - Formatted as mm/dd/yyyy

License number - Consists of six to eight alphanumeric characters (e.g., 123456 or E0A12345). Must match exactly, no spaces. Must match exactly, no spaces.
Apostrophes: Apostrophes, commas and periods can be part of the licensee’s name (e.g., D’Angel, O’Brien, Inc.), and will be required if so.
Names with Spaces: Spaces in the name matters. If the last name is Mc Donald, type the last name with a space, and then type the name without a space. For example: "Mc Donald" and then "McDonald".

FEIN - Entire Federal Employee Identification Number (FEIN) is required. It is a 9 digit numeric field and a dash is optional.

Business Entity Name - Enter a CDI approved business name (i.e. legal name or dba name). Must match exactly, no partial name searches, not case sensitive, and alphanumeric.

License number - Consists of six to eight alphanumeric characters (e.g., 123456 or E0A12345). Must match exactly, no spaces.
Apostrophes: Apostrophes, commas and periods can be part of the business name (e.g., Firemen's or Lawyers' Mutual, Inc.), and will be required if so.
Names with Spaces: Spaces in the name matters. If the business entity name contains Mc Donald, type the name with a space, and then type the name without a space. For example: "Mc Donald" and then "McDonald".
